Особливості респіраторної підтримки методом неінвазивної вентиляції у періоді новонародженості: міжнародний досвід

Анотація

Неінвазивна вентиляція в структурі лікування недоношених дітей є важливою ланкою в процесі виходжування цієї групи пацієнтів. Сучасна практика дитячого анестезіолога потребує більшої кількості досліджень різних режимів неінвазивної вентиляції. Загалом в широку практику неінвазивна вентиляція була залучена в останні 11 років. Саме за цей період проводився порівняльний аналіз досліджень. Основним методом пошуку була стратифікація досліджень за метою, розміром виборок та результатами: частота реінтубації, розвиток бронхолегеневої дисплазії, некротизуючого ентероколіту, летальність, тривалість знаходження пацієнтів на ліжку інтенсивної терапії. Основні джерела були отримані із баз даних Національних бібліотек Європи (найбільший відсоток досліджень іспанські, французькі та англійські), Національної медичної академії Америки, ряду науко-метричних джерел України методом PubMed пошуку, та як результат патентного пошуку. Недостатньо для достовірності мультицентрових рандомізованих досліджень в даному питанні у зв’язку із невеликим обсягом виборок. Не було проведено жодного дослідження щодо впливу неінвазивної вентиляції на частоту розвитку бронхолегеневої дисплазії, також ряд досліджень не є достовірними, тому що в аналізі не було враховано великої розбіжності між фізіологією патологічного процесу у недоношених різного строку гестації. Даний огляд дозволяє співставити два основних метода неінвазивної вентиляції у недоношених дітей, залучити ці методи у своїй практиці та знайти в цьому напрямку можливості дослідження, оскільки багато питань залишаються відкритими. Також даний огляд висвітлює основні особливості використання неінвазвиної вентиляції у цієї особливої групи хворих.
