ВАТС: от однолегочной ИВЛ к «nonintubated» и «awake» хирургии

Анотація

С начала 1990-х годов в мире быстро развивается и широко применяется видеоассистированная торакоскопическая хирургия (ВАТС), охватывающая практически все области торакальной хирургии. Минимально инвазивный подход продемонстрировал превосходство в сокращении продолжительности пребывания пациентов в стационаре, облегчении послеоперационной боли, улучшении постоперационной функции легких и снижение общей заболеваемости после операции. Хотя общая анестезия с эндобронхиальной интубацией и проведением однолегочной вентиляции легких все ещё является «золотым» стандартом при выполнении ВАТС хирургических вмешательств, интерес к альтернативным методам анестезиологического обеспечения всё больше и больше возрастает в последние годы. Этому способствовали накопленные данные о риске развития возможных осложнений после общей анестезии, включающих травму дыхательных путей, связанную с интубацией, вентиляционное повреждение легких, остаточную нервно-мышечную блокаду, нарушение сердечной деятельности, послеоперационную тошноту и рвоту. В последние десятилетия выполняются и изучаются неинтубируемые видеоторакоскопические хирургические вмешательства, так называемые nonintubated VATS (НИВАТС). При проведении НИВАТС к пациентам применяются различные протоколы регионарной анестезии с сохранением спонтанного дыхания, с или без седации. При согласованной профессиональной работе хирургической и анестезиологической бригады, тщательном отборе пациентов, надлежащем мониторинге, грамотном использовании регионарной анестезии, седации и дыхательной поддержки, этот способ анестезиологического обеспечения оказался более безопасной альтернативой обычной общей анестезии с интубацией трахеи.
