Періопераційна серцево-легенева реанімація

В статті наведений огляд проекту гайдлайнів з періопераційної серцево-легеневої реанімації, який запропонований міжнародною групою експертів та аналіз ситуації в Україні. За даними річних звітів анестезіологічної служби 20 областей України за 2017 рік зареєстровано 116 випадків зупинок кровообігу в операційній та за першу добу після операції. Зареєстровано 38168 випадків ускладнень в інтраопераційному періоді та протягом першої доби після операції, 96 (0.25%) з яких закінчились смертельно. Загальна кількість проведених анестезій за 2017 рік складає 858331. Інцидентність зупинок кровообігу в операційній 1:7400 анестезій, смертей 1:8940 анестезій. Такі занижені показники (порівняно з даними розвинених країн) скоріш за все є наслідком некоректної реєстрації та не відображає об’ективної ситуації. В Україні забезпеченість мінімальним моніторингом складає 0.46 на 1 операційний стіл. Катастрофічно не вистачає наркозних станцій, сучасного моніторингу та оснащення на випадок «складних дихальних шляхів». Проведення інгаляційної анестезії в режимі малого/мінімального потоку без мультигазового аналізатора стало звичайною практикою в Україні. Майже абсолютно відсутній моніторинг нейро-м’язової провідності, глибини наркозного сну.
